Care Coordinator (Registered Nurse) Tamworth

Reporting to the Clinical Care Manager NSW you will be responsible for assisting with the provision of quality services through assessment, case management, monitoring and review of services for eligible clients in accordance with McLean Care Ltd values and policies. You will provide holistic nursing care based on assessed client needs in accordance with the organisation policy and procedures. In this position you will provide leadership, support and guidance to other members of the team to enable the provision of quality service to enhance the lives of clients.

Essential Skills

  • Ability to balance competing priorities of individual’s expectations against resource levels of the program and communicate these to clients.
  • Demonstrated ability to seek out and effectively utilize new knowledge as it relates to nursing practices.
  • Ability to devise creative responses to human needs based on use of formal services and informal networks.
  • High level interpersonal and negotiation skills including the ability to provide support particularly for people experiencing change in their later life. 
  • Ability to work effectively and cooperatively as a member of a team.
  • Ability to assist clients to formulate plan of care within budget and report all income and expenditure to clients on a regular basis.
  • Excellent Microsoft Office Suite skills.

Knowledge

  • An understanding of the ageing process with comprehensive clinical knowledge, particularly to age related health issues. 
  • Demonstrated experience in clinical assessment and care planning that identifies appropriate interventions to optimize the outcomes for consumers. 
  • Demonstrated experience in assessment, referral, and creation/coordination of human services.
  • Demonstrated experience in guiding and supporting healthcare workers and enrolled nurses. 

Essential Qualifications

  • Current APHRA registration as a Registered Nurse
  • CPR 
  • Current unencumbered Driver’s License and own insured/registered vehicle
