Senior Independence and Wellbeing Practitioner

  • £32,076 per annum (pro rata if part time)
  • 37 hours per week, working one Saturday a month

A great opportunity has arisen to join Suffolk County Council as a Senior Independence and Wellbeing Practitioner working from Ipswich Hospital within Adult Social Care. 

Your role and responsibilities 

You will work as part of the Adult Social Care team using the discharge to assess model alongside health partners in Ipswich Hospital. This post will require you to work on site and doesn't offer any hybrid options.

In this role, you will get the support you need to develop your skills and knowledge in adult social care. Working closely with social workers, and other Independence and Wellbeing Practitioners and staff from across the locality to get the best outcomes for people.

Being involved with using digital care solutions to assess and support people, you will actively contribute to the ongoing development of the service. This will give you the opportunity to progress your career through applying to the social work apprenticeship scheme or occupational therapy apprenticeship scheme. 

You will be reviewing people in hospital and facilitating their hospital discharges. You will need excellent communication skills with the people you are working alongside and the motivation to contribute to a multi-agency approach for responding within urgent timescales.

This team seeks to improve the quality of life and independence of the people we work with.  Within the hospital we continue to develop changes to the discharge processes.  These will minimise a person’s stay in hospital whilst maximising the opportunities our teams have to work with people in a community setting focusing on reablement and improving their health and long-term well-being.
