We care for people, family and friends and help to manage complex issues a person with a disability experiences every day; we work with families and communities to enable individuals accessing care choice and control over their lifestyle while we provide post-acute rehabilitative care in their homes.

Holding the person-centered approach in the highest regard, we consider the safety, health and well-being of the person and their families who we support, and ourselves. It is our priority to understand and address each individual's specific needs. This allows for transparency with communication and sharing of information, setting goals together, and then taking actions to achieve those goals.

As an integral part of the interdisciplinary team, we advocate and promote every person’s independence through building capacity for self-care which leads to confidence and a feeling of accomplishment.
