75522Residential In Reach - Clinical LeadSenior Nursing Role - Clinical LeadershipPermanent Role – Full Time hours with ADOMake a difference to people living in aged careThe Residential In Reach (In Reach) service aims to reduce unnecessary presentations to the Emergency Department by providing acute support, early intervention and patient focused healthcare within Residential Aged Care Facilities.The service is staffed by a team of clinical nurse consultants, with expertise in the areas of gerontology and emergency care, as well as Geriatrician clinical lead and Medical staff.The clinical lead role of the Residential In Reach team provides visible and accessible clinical leadership to the clinical nurse consultant group and leadership and management of the service.
