The Aged Care Assessment Team at the Mackay Base Hospital are seeking applications for a Senior Health Practitioner to join their team on a permanent basis.

Your opportunity

As an Assessor in Mackay Aged Care Assessment Team (ACAT), you will undertake comprehensive assessments of aged clients in a variety of settings, both community and hospital. Your primary objective will be to record accurate data collected during the comprehensive needs-based assessments of clients using the National Screening and Assessment Form (NSAF) within the myAssessor application and the MyAgedCare client record. Within a Wellness and Reablement framework, the assessor will develop a client centred Support Plan detailing client recommendations and aged care and community pathways to improve service provision and agreed outcomes. The assessor is responsible for providing up to date information on appropriate aged care pathways that the client may be eligible to access. As an assessor you will liaise with clients, carers, health professionals, service providers and other internal and external stakeholders.

About the Team

The Aged Care Assessment Team is a small, highly skilled multidisciplinary team (MDT) consisting of nursing and allied health clinicians whose primary role is the assessment of frail aged clients 65 years and over and Indigenous clients 50 years and over seeking to access Government subsidised services to assist them to maintain their independence at home, or for entering aged care residential settings.
