Rehabilitation Family Physician (100-1887) Reference Number 100-1887 There is an opportunity for physicians to join this new exciting service and be part of a multidisciplinary team in providing MRP care to patients admitted to one of two units: a Low Intensity Inpatient Rehabilitation Care Unit for patients returning home after prolonged hospitalization, and an Alternate Level of Care Unit (ALC) for patients awaiting placement in LTC. Part time and full time scheduled hours are required to meet the unit hours of 8-4pm; these admitted patients also require after-hours/stat/weekend coverage, on a rotating basis, to be provided by the physician group. Each Unit provides care to a population of up to 45 patients, and include Complex Medical Frail and (typically) an older adult population with orthopedic and neurologic conditions. Each physician on the team will provide on-site weekday direct patient care including completion of new patient admissions, on-going diagnostics, assessment and follow up, as well as patient discharge planning to support the continuity of patient care, in consultation with the patients’ family physician following discharge. Each physician will also contribute to after hours coverage of both Units. This position would be ideal for a physician seeking a multidisciplinary team work environment, who is familiar with a complex medical and frail patient population, and particularly a physician who provides service in long term facilities but seeks to provide team-based care to a population on one site, the Gorge Road Rehab site.
