We are currently seeking a dedicated Care Coordinator  to join our team at the Katherine Support Office. This is a full-time, fixed-term position that will involve regular remote travel to communities. As our Care Coordinator, you will be an integral part of our Integrated Health Team , working collaboratively with patients, general practitioners, clinical/practise staff, and other relevant services to facilitate the provision of appropriate multidisciplinary care for Aboriginal people with chronic disease.

Position Responsibilities:

  • Managing referrals, enrolment, support, and client exits from the ITC Programme.
  • Implementing and embedding the annual work plan for the ITC program
  • Liaising with care providers to enhance the quality and consistency of follow-up care for enrolled ITC program patients
  • Delivering regularly scheduled visits to enrolled clients, aiding in developing self-management skills and improving health literacy
  • Actively supporting Chronic Disease portfolio holders in each remote clinical team
  • Monitoring the implementation of Care Plans by primary healthcare teams
  • Recording all work on the Communicare patient information system and other systems as required

What You'll Need to Succeed:

To be considered for this role, you will hold a relevant qualification and/or registration as an Aboriginal Health Practitioner Certificate IV  or be a Registered Nurse with current AHPRA registration . Candidates will have a demonstrated knowledge of or willingness to learn about Primary Health Care Principles and their application in a remote Aboriginal community setting. Experience in delivering chronic disease self-management training, healthcare or health literacy in a cross-cultural setting will be an asset.

The ideal candidate for this position will have:

  • Current unrestricted manual driver's Licence.
  • Current First Aid Certificate.
  • Non-adverse National Criminal History Check.
  • Current OCHRE clearance.
  • Acceptable vaccination status to work with vulnerable people in the NT.
  • Highly developed communication and interpersonal skills.
  • Ability to work independently under limited direction, prioritise workload, and manage time effectively.
  • Working knowledge of the Medicare System with respect to Chronic Disease.
