About the Role

As our Clinical Case Manager Lead you will be responsible for the clinical and administrative oversight of Clinical Case Management of Home Care Packages at The Landings. You will ensure that clinical case management and reporting requirements are up to date and oversee integrated and coordinated resident care and support. Your responsibilities include (But not limited to):

  • Assist key team members in relation to care escalation and reporting
  • Oversight of each of the Clinical Case Managers portfolios
  • Assist each of the Clinical Case Managers with onboarding new residents and ensuring they understand their roles
  • Have an understanding of and be able to identify eligibility for government funded care
  • Provide training and support of team members in relation to Clinical Case Management
  • Provide guidance and oversight to Clinical Case Managers to liaise with the resident/representative about eligibility for accessing externally funded care
  • Coordinate aged care assessment through ACAT/MyAgedCare

About You

Qualifications

  • Current Registration with APRHA as a Registered Nurse

Professional Experience

  • Experience in the community care sector/home care package management
  • Previous aged care nursing experience
  • Highly developed interpersonal and communication skills.
  • Experience working as a member of a multidisciplinary team
  • The ability to liaise with key stakeholders (e.g., consumers, families, multidisciplinary teams, LDK management and other providers).
  • Ability to focus on resident goals and promotion of independence and re-ablement
