DEPARTMENT
The Endocrinology and Diabetes department at Alfred Health is dedicated to providing the highest standard of evidence-based care for individuals living with diabetes. At the heart of our service is a multidisciplinary team of highly skilled healthcare professionals who work collaboratively to provide personalised care addressing the unique needs and goals of each individual. Our knowledgeable team are compassionate and empathetic, taking time to listen, educate, and empower individuals to actively participate in their diabetes management, promoting a sense of ownership over their health.Our team is committed to supporting our patients in achieving optimal health and improving their quality of life. POSITION
This is a new Diabetes Nurse Practitioner (NP) role within the recently established dedicated Endocrine & Diabetes Type 1 Service . The NPs primary focus is to improve the health outcomes of people with type 1 diabetes, especially those using technology including insulin pumps and continuous glucose monitoring. The Diabetes NP role isintricately designed to address the dynamic demands faced by the Endocrine & Diabetes service, operating both autonomously and within a multidisciplinary framework. The Diabetes NP is responsible for the provision of advanced clinical practice and is empowered to provide timely, high-quality care, involving advanced clinical assessments, medication prescription, diagnostic orders, ongoing treatment, and the capability to refer patients to specialists. This practice is firmly rooted in evidence-based principles, utilizing resources such as Alfred Health clinical guidelines and expert-endorsed practice guidelines. Operating within both autonomous and collaborative care models, the Diabetes NP takes professional responsibility for the entire patient care episode in autonomous scenarios and consults senior medical colleagues in collaborative decision-making. The degree of autonomy is carefully determined by factors such as patient age, clinical complexity, and co-morbidity. In cases without significant clinical complexity, the Diabetes NP is authorised to autonomously assess, diagnose, and treat patients.
