Superior Home Health Services are seeking a qualified, reliable and passionate Home and Community Registered Nurse - Clinical Advisor to join our team at the North Lakes - Head Office.

Responsibilities will include:

• Working within a person-centered, enabling environment that respects the rights and needs of each client;
• Manage case management for a variety of aged and disability clients;
• Manage Home Care Package client intake assessments;
• Oversee medication management and wound management activities;
• Observe, care and report any changes in the health and wellbeing of the client;
• Undertake skilled nursing tasks where required;
• Responding to emergency situations
• Bachelor of Nursing
• Excellent verbal and written communication skills
• Willingness to learn, develop and take on new challenges
• Current National Police Check
• Current First Aid & CPR certificates
• NDIS Worker Screening including Blue Card (or equivalent)
• Community experience, 2 year minimum

Preferred Criteria

• Previous Care Coordination/Case Management in Community
• Post Grad qualifications and specialties relevant to community care
