SVVH Care Coordinator

Our St Vincent's Virtual & Home program is growing, providing the opportunity for more people to receive high quality clinical care at home. Integrated with the St Vincent’s hospital and community care network we are a multidisciplinary team of physicians, allied health, nursing and personal care workers supporting individuals to leave hospital sooner, remain in their homes and achieve their goals. About the Role

We are looking for an enthusiastic Care Coordinator (Registered Nurse) with broad experience in the health care sector to join our Rehabilitation at Home team. Be part of a growing service that supports hospital demand management and meets individual preferences for quality care at home.

On a daily basis you will:

  • Provide direct client care and care coordination ensuring that services are provided in accordance with the Rehabilitation at Home model of care
  • Manage the program’s intake function including cohort allocation and oversight
  • Work collaboratively with a wide range of stakeholders including SVVH colleagues and external service providers
  • Contribute to the development and delivery of virtual care solutions nationally
  • Drive improvement initiatives and service development activities to support the growth of the program nationally

Your Contribution

  • Relevant clinical experience in acute and/or community setting
  • Current nursing registration with AHPRA, as well as a current Victorian motor vehicle license are essential
