Position Summary

Within the context of patient centered care model and in accordance with the established vision, values, and ethical standards of Northern Health and in accordance with the standards of practice and code of ethics as outlined by the BC College of Nurses and Midwives (BCCNM), the Community Integration Care Coordinator (CICC) acts in a senior supervisory role and serves as a role model and resource to staff for decision support, including clinical decision making. The CICC is responsible for the oversight, monitoring, support, and guidance of discharges from acute care to community services. Supports the seamless movement of clients through the health care system and in accessing appropriate resources. The CICC will liaise with primary care and interdisciplinary teams for identification, coordination, and efficient utilization of community resources. The position requires an in-depth knowledge of community processes, resource allocation, and policies pertaining to community service delivery.

Shift Rotation/Hours of work: Days, Evenings - 07:00 to 15:00, 15:00 to 23:00, Rotating

• Patient care addition with an expanded emergency room and updated ICU
• State-of-the-art maternal-child care centre (including a single room neonatal ICU)
• Facilities to accommodate undergraduate physician training
• MRI unit
UHNBC is also home to a cancer centre for Northern BC, featuring a wide range of cancer treatments including radiation therapy.

Qualifications

• Bachelor of Science Nursing (BScN) from an approved School of Nursing with current practicing registration with the British Columbia College of Nurses and Midwives (BCCNM).
• Three years of recent related clinical experience in community nursing and two years recent supervisory experience or an equivalent combination of education, training, and experience.
• Current valid BC Driver’s License

• Teaching – Ability to transmit information intended to instruct clients and others about topics essential to health care and well-being.
• Assessment and Intervention – Demonstrated ability to complete initial and ongoing client assessments (clinical and diagnostic reasoning) and provide nursing care through appropriate/ prescribed technical, therapeutic, safety type interventions.
• Ability to operate related equipment including proven ability to utilize computer technology.
• Physical ability to perform the duties of the position.
