Woodrow Mercer Healthcare are currently recruiting for a Clinical Patient Pathway Coordinator to support a healthcare provider based in Manchester, on a permanent position.


Job Purpose

  • The Clinical Patient Pathway Co-Ordinator will work within a wider team supporting the various specialties with all aspects of clinical pathway management.
  • Facilitating the smooth running of the patient pathway by ensuring that they are coordinated and streamlined.
  • Ensuring pathways are clinically appropriate and revolve around the patient and their individual needs, proactively monitoring, liaising with consultants and surgical teams.


As this role forms part of a wider team of within elective surgery, there will be an expectation of being able to flex activity with various specialties as well as undertake a wide range of pathway related duties such as coordination and direct liaison with patients and various teams.


Job Responsibilities

  • To navigate patients through their care pathway from point of referral through to treatment in accordance with locally and nationally defined timescales
  • To ensure that clinical processes and procedures are followed in accordance with best practice and national guidelines.
  • To be responsible for educating patients and their families, creating and implementing a care plan that meets the patient’s needs, regularly reviewing the care plan with patients and monitoring their progress, and maintaining patient medical records.
  • To be responsible for communicating and coordinating with other direct care providers
  • To be responsible for overseeing all day-to-day clinical activities at a healthcare facility, evaluating personnel, and preparing daily reports as needed by other departments, and assisting with recruitment, consenting, screening and enrollment of personnel.
  • To pro-actively track appointments/admissions to ensure that patients reach each step of their pathway within locally and nationally defined timescales.
  • To escalate problems with capacity and pathways to colleagues to support/provide a solution
  • To escalate diagnostic and other appointment/admissions booked outside of the appropriate timescale.
