About the role

The Cancer Care Coordinator (CCC) role is designed to complement existing nursing services provided by West Moreton Health. The CCC provides specialised Lung and Skin Cancer (Melanoma) cancer information, education, and support to patients with these cancers.  The applicant will collaborate with multidisciplinary team members and relevant community service providers to coordinate delivery of specialized, support services across the care continuum to patients and their families.

Within this role the CCC will be responsible and accountable for providing a high level of person-centred supportive care for patients with complex health needs. This encompasses patient assessment and coordination of care required, provision of education to patient, carer, and staff; consultation, provision of counselling to the patient and carer, and liaison and collaboration with multiple stakeholders to ensure the best patient outcomes. The CCC is an integral and active member of the multidisciplinary team and hold reporting and data collection responsibilities.

The CCC is an advanced practice registered nurse who provides support and nursing leadership within West Moreton HHS. This position provides expert knowledge, skill and abilities for clinical care and guides clinical practice across this service to support patient centred care throughout their cancer journey.  This position also supports the Nurse Unit Manager to meet the strategic and operational objectives of patient-centred care.

About you

Our ideal candidate will be someone who can demonstrate the following:

  • Demonstrated ability to apply specialised knowledge of contemporary nursing practice and theory in Lung & Skin (Melanoma) Cancers and participate directly and indirectly in the delivery of patient centred consultancy and clinical care with the ability to work independently with a high degree of initiative.
  • Demonstrated excellence in communication and inter-personal skills across a variety of mediums and ability to build and maintain strong relationships, open communication, and a positive environment within a multidisciplinary team inclusive of external stakeholders.
  • Proven ability to implement changes and develop direction in clinical practice by the utilisation of evidence-based research and best practice principles.
  • Demonstrated skills in strategic and clinical service planning including an understanding of organisational behaviour, change management and cultural change processes.
  • Demonstrated skills in clinical teaching, competency in the use of contemporary information systems and experience in the development of specialty education programs.
