Aberdeen City OOH Community Nursing, Band 6 District Nurse

An opportunity has arisen for a Band 6 District Nurse to join our Out of Hours Community Nursing

Unscheduled Care Team within Aberdeen City HSCP.

You will be managing and supporting teams, who work within an 8 day rota, delivering an unscheduled clinical nursing service overnight, responding to referrals from GMED, NHS24 and other professionals, prioritising end of life care and symptom control for patients in their own homes.

Candidates must possess a Specialist Practitioner District Nursing qualification and should evidence current clinical and workload planning experience at Band 6. Team Management experience is essential to the role, as well as current experience in palliative and end of life care.

2300 – 0800: 3 fixed  nights   (25.5 hours weekly).

Aberdeen City OOH Band 6 Senior Community Nurse

There is also an opportunity for Band 6 Senior Community Nurses to join the OOH team

The OOH Senior Community Nurse will work with and support Band 5 and Band 3 colleagues delivering a person centred clinical nursing service, prioritising end of life care and symptom control to enable patients to remain in their own homes.

The role includes triage and management of unscheduled nursing referrals from GMeds, NHS 24, Hospital at Home and other professionals.

Candidates must possess a Graduate Diploma in Integrated Community Nursing, or a relevant Post Graduate Certificate and evidence current clinical experience in a Community Nursing role, which includes triaging of referrals, workload planning, palliative and end of life care experience

1800-0000: working 3 shifts ON 5 shifts OFF within an 8-day rota (16 hours per week pro rata)

Car driver/owner essential for the role. ( LEZ compliant ).  Current clean UK driving licence required.
