We have a highly engaged, diverse and collaborative workforce that is unified by our purpose, which is to help people live healthy and fulfilled lives.The HIP Chronic Heart Failure Care Management service:Works with community-based clients by assisting them with the management of their heart failure and includes:Advanced Practice Heart Failure Nurses who provide support, information and assistance with the navigation of involved health care services,Education for clients and carers to encourage client self-monitoring and self-management.Liaison with GPs and Cardiologists.Phone and home visit follow up after a hospital inpatient discharge.About youTo be successful in this role you'll bring with you:substantial clinical experience in the management of people with heart failure,post-graduate qualification in Critical Care, Coronary Care, or equivalent,sensitivity to the needs of of people from culturally and linguistically diverse backgrounds,current registration with AHPRA,Working at Austin Health means enjoying a strong sense of purpose, engaging in meaningful work every day.
