Nursing - General Medical & Surgical (Healthcare & Medical)

We are looking for a casual, experienced Chronic Disease Management (CDM) Nurse to join our Gladstone based Practice. This role will see you support GP’s and their patients to identify and manage their chronic conditions and provide Health Assessments for eligible patients. This role will start at 24 hours a week, with the possibility to transition to a permanent position based on the business needs.

Established in 2010, we consider ourselves a one-stop shop, offering access to experienced General Practitioners, an amazing team of nursing staff and a range of allied health professionals.

Gladstone GP Superclinic is one of 12 GP medical centres owned and managed by Ausdocs Group. Established in 2018, the group is expecting to continue growing at a steady rate, with additional medical centres across the Eastern seaboard becoming part of the expanding Ausdocs Group family over the coming years.

Duties

• Preparation of Care Plans and Team Care Arrangements
• Assist patients to access ongoing care from a multi-disciplinary team
• Conduct health assessments to assist in early identification of chronic diseases
• Promote preventative healthcare throughout the practice
• Assist management in driving chronic disease health care in our Practices
• Work with other Practices in our group to provide remote preparation of plans

The Successful Candidate

• AHPRA registered Enrolled or Registered Nurse
• Proven prior experience in Chronic Disease Management and Health Assessments 
• Strong organisational and time management
