About the role

  • This role will provide specialist geriatric assessment, discharge support, acute and subacute management, and monitoring for patients with behavioural and psychological symptoms of dementia and other geriatric syndromes within the community and Residential Aged Care Facilities (RACFs) of Metro North.
  • The service is an outreach and telehealth acute inpatient, outpatient and ED substitution model. ▪ The role will identify and facilitate the care of eligible long stay patients in a local RACF as part of an incentive payment model of care.
  • The role will also support RADAR RDH/CBH teams and OPEN teams as needed.
  • In association with the Director, Staff Specialists, Nurse Navigators, Nurse Practitioners and Registered Nurses, provide clinical care and supervision of the OPEN, RADAR Redcliffe Hospital/Caboolture Hospital (RDH/CBH) and OPEN Dementia and Delirium Intervention Service teams as needed.

  • Trauma Management - basic competence in trauma management, general / inpatient, and emergency medicine as relevant to the granted scope of clinical practice (SoCP).
  • Multidisciplinary Team - ability to function in a multidisciplinary team and provide leadership within the team
  • Ability to provide expert specialist acute and patient focussed care to frail elderly patients.
  • Evidence of leadership skills and the ability to liaise with multi-disciplinary professional staff.
  • Ability to participate in the education and supervision of Registrars and Resident Medical Officers.
  • Have a working knowledge of ACEM and RACP training requirements as relevant, including work-based assessments and the mentorship program.
  • Ability to undertake research and participate in undergraduate and post graduate teaching.
  • General registration as a medical practitioner or specialist registration as RACP in Geriatrics with the Australian health Practitioner Regulation Agency (AHPRA). Certified copies of the required information must be provided to the appropriate supervisor/manager prior to the commencement of clinical duties.
  • Fellowship with RACP in Geriatrics is preferred
  • Additional fellowship with RACP in General and Acute Care Medicine will be highly regarded
