Total Indicative Remuneration Salary (Subject to private practice agreement)Multiple Part/Full Time, Contracted Roles available up to 12 monthsAbout the roleIntegrated Care (IC) provides health services including, but not limited to, planned care, rapid assessment clinics, Virtual care and phone advice/triage to support clinical decision making, with a focus on reducing patient length of stay in acute settings, prevent hospital presentations, and bridging the gap between primary and tertiary care.The medical consultant role is accountable to the Medical Head of Unit, IC and is responsible for managing the care of all patients requiring the services of Integrated Care. The position works in partnership with acute, sub-acute, community and primary care teams to ensure patients are both cared for and navigated through the system based on clinical need. Intervention will be for those assessed as requiring a rapid / time limited intermediate health care response only. Those assessed as requiring inpatient or emergency care will receive care via alternate pathways.The position works as part of a multidisciplinary team comprising medical, nursing, allied health and administrative positions. The incumbent actively contributes to continuous quality improvement in the interests of effective and efficient patient care and with a commitment to clinical teaching.IC services operates as a same-day (not overnight) services.
