About you:

In your new role you will…

• Demonstrate knowledge and skills in assessment and intervention for older persons and/or adults in community settings, including a demonstrated commitment to professional development and evidence-based practice.
• Demonstrate knowledge and (preferred) experience in case management.
• Demonstrate ability to identify, lead, implement and evaluate quality improvement initiatives and provide advice to professional and operational supervisors.

About your opportunity:

In this position you will deliver safe and effective occupational therapy services to consumers of the Rapid Response Falls Service, their partners, carers and families, as part of an interprofessional team. . This position may involve working in other clinical service areas within Adult Community Health Service as guided by service demands.

About your role:

• Provide consistent, safe and efficient occupational therapy services and case management when required, within an interprofessional team, under clinical practice supervision commensurate with experience.
• Direct allied health assistants when required in carrying out clinical and non-clinical occupational therapy tasks under the endorsed delegation model.
• Fulfill the responsibilities of the Rapid Response Falls Service adhering to procedural instructions and directives.
• Effectively communicate with professional and operational supervisors, and internal and external stakeholders to optimise clinical services and consumer experience.
