About the role

  • The purpose of this role is to deliver advanced level, evidenced based Social Work services and coordination of care for consumers of the Mental Health Intensive Discharge Team (MIND), their carers and relevant stakeholders, as part of an interprofessional team to expediate discharge from Metro North Mental Health Facilities.

In Terms of technical knowledge, skills and experience within the context of responsibilities described above under Key Accountabilities the ideal applicant will be someone who can demonstrate the following:

Clinical Expertise: Demonstrated advanced level of knowledge, skills and expertise in social work clinical practice in the field of complex psychosocial disability and presentations of challenging behaviours.  

Communication/Team participation: Demonstrated advanced ability to communicate and participate effectively as a member of an interprofessional team, including providing expert clinical advice to external service providers, clinicians, and family/carers of clients, and effective leadership in application of conflict management.  

Leadership: Demonstrated ability to provide professional and clinical leadership within the MIND team, including, clinical supervision and support for less experienced practitioners and students, setting, maintaining and evaluating clinical practice standards.    

Quality improvement: Demonstrated ability to lead, implement and evaluate service improvement initiatives through continuous quality improvement, knowledge translation and research projects that enhance outcomes for consumers of MIND and influence clinical practice standards across Queensland.
