About the Transition and Restorative Care Programs

The Transition and Restorative Care programs sit within Aged Care Community Services as part of the continuing Care Division of Austin Health. The aim of Transition Care is to provide short term support for older people at risk of inappropriate extended hospital stays and/or at risk of premature admission to residential care. Restorative Care is a subset of Geriatric Evaluation Management (GEM). The programs are committed to providing high quality safe care for our patients while helping them achieve their goals. The programs are provided in both residential and community settings, with our residential beds being in a local Residential Aged Care facility.

About the role

As the Grade 3 Physiotherapist you will be recognised as a clinical leader in the Transition Care and Restorative Care programs. You will demonstrate high level specialist knowledge and clinical skills in the areas of gerontology, chronic and complex conditions, with an ability to motivate and inspire others.

 A regular day will see you:

  • Work collaboratively with the Community Aged Care Services Manager and Transition Care Team Leader to oversee the development and clinical service provided by the Allied health team.
  • Lead and instigate clinical service improvement and innovation activities, and assist to evaluate resource utilisation within the multidisciplinary allied health team. 
  • Provide operational support to the Allied Health Team and clinical supervision to our Grade 2 physiotherapists.
  • Assess and treat patients, liaising as appropriate with team & family members/ caregivers to support achievement of goals and discharge planning.
  • Work collaboratively with the wider Transition and Restorative Care Team members to ensure high quality patient care and support organisational bed flow.

About you

You will be someone who has excellent communication skills, with a strong commitment to excellence in patient care. You’ll bring energy and drive to your work and have a keen interest in developing your skillset.

You’ll further have:

  • A physiotherapy qualification and current AHPRA registration
  • Extensive clinical experience in aged care/gerontology, working across inpatient and community settings.
  • Well developed interpersonal skills that promote team development and engagement, and a demonstrated ability to engage with a broad range of internal and external stakeholders.
  • Strong clinical supervision skills
  • Demonstrated ability to identify opportunities for process improvement or redesign
