Hi there, we're Five Good Friends, and we're a new, modern and fast growing approved aged care and registered NDIS service provider. We help our clients, we call them Members, live vibrantly in their own homes by wrapping a Care Team around them and their families.

To do this well we need to provide great support along every step of their journey. Today we are looking for the right people to support some of their first steps. About the Role

As a Community Nurse you will visit and support people and their loves ones who are looking for the right provider of their care and support. You’ll help them understand how they can use their funding with Five Good Friends, whether on a Home Care Package, with a NDIS support plan and how we can work together to support them to live the life they choose.
 

Working with each individual you will complete a comprehensive assessment using our online assessment tool to create a story that captures their needs, goals and preferences. You will work together to understand what is important to them, what they need and want to stay living at home, and begin to plan how we can support this.
You’ll be an advocate for the use of our software, supporting Members and their families to use our FGF app to ensure they have access to everything they need.
This is the beginning of the onboarding journey with Five Good Friends and is critical for the development of great Help Plans and sourcing the perfect Helpers (care workers) to build a great relationship. Doing this well will provide a great experience and smooth transition into our Welcome Team and ultimately into ongoing care and support with Five Good Friends.

Experience and qualifications:

  • Formal qualifications in nursing is a must
  • Current or recent community, aged or primary care nursing would be highly regarded, however, we know how broad nursing is and how great many different people can be
  • Strong and genuine interpersonal, oral and written communication and negotiation skills are important - you need to be able to talk with and support many different people, and handover top-quality information to your colleagues 
  • Understanding and experience in providing excellent customer service is valuable to us
  • Experience in Wound and Catheter management is advantageous
  • Current driver's license
  • Successful National Police Check (we can do this for you)
  • NDIS National Worker screening check (we can do this for you)
  • Current First Aid and CPR certification


Work flexibly 

You'll have a flexible work arrangement allowing you to work from home when you are not visiting our Members. At times people may need our support people outside of usual business hours so some flexibility on your part is important.
