About us: 

The Community Integrated Falls Service (Cairns) is a falls prevention initiative that aims to identify and implement an integrated pathway for at-risk individuals living in the Cairns region, to access appropriate rapid in-home assessment, tailored interventions, and linkages to existing services to reduce risk of falls and reduce conveyance to emergency departments.  The service will strengthen sustainable partnerships between Cairns and Hinterland HHS and the primary and community care sector.

About you: 

You are a qualified Registered Nurse with advanced practice skills relating to Adult Care in community settings, ability to provide strategic and operational management to a multidisciplinary team. You possess advanced interpersonal, negotiation, consultation, and conflict resolution skills, and the ability to effectively communicate verbally and in writing to senior managers, stakeholders, multidisciplinary team, and target client group.

Your role: 

  • Assist the Adult Community Health Team Leader to monitor the organisation's quality standards, work health and safety policies, procedures and programs and provide clinical governance in the relevant work area.
  • Manage the CIFS (Cairns) referrals and Intake process.
  • Build on existing and develop new partnerships and promote service to key stakeholders and provide feedback/recommendations to CIFS Working Group prior to commencement in early 2024.
  • Support development of CIFS referral and service provision pathways.
  • Work in partnership with QAS. Position to be co-located at Queensland Ambulance Service (to be confirmed).
  • Work directly with the CIFS multidisciplinary team to provide assessment and intervention to eligible persons referred to service.
  • Complete relevant onward referrals to existing services and provide correspondence to service providers at the end of the participants CIFS (Cairns) episode.
