Key Responsibilities

  • Conducting thorough assessments of residents' clinical needs, which includes monitoring vital signs and relevant health indicators, to provide essential treatments and procedures with precision in medication administration, nutrition, hydration, and proficient management of pressure care.
  • Recognizing and promptly documenting changes in residents' health status to enable timely interventions, while also offering expert advice on wound management and implementing tailored strategies to optimize care for diabetes and other chronic conditions.
  • Establishing strong communication channels with residents, families, and healthcare professionals to address evolving needs, and implementing behavioral management strategies to enhance residents' well-being and comfort.
  • Adhering to legislation, regulations, and guidelines while making informed decisions within the scope of professional practice, maintaining accurate and comprehensive documentation of assessments, plans, and interventions, and advocating for residents' autonomy, fostering professional relationships, and promoting a culture of safety and ongoing learning for person-centered care.
Job Requirements
  • Minimum of one year of clinical experience specialising in individualised care.
  • Proficiency in assessing needs, developing care plans, and adhering to regulatory guidelines.
  • Excellent verbal and written communication skills.
  • Competency in computer usage and effective time management.
  • Comprehensive understanding of safety protocols, infection control measures, and proper lifting techniques.
  • Demonstrated leadership experience, including staff management, within a healthcare environment.
  • Familiarity with providing care for older adults or similar populations.
  • Current registration with AHPRA.
