Tell me more...The Clinical Nurse Consultant (CNC) is required to deliver advanced Primary Health Care while to promote and support health and wellbeing in those who experience chronic illness. This position requires the utilisation of an integrated health delivery approach and partnering with other government and non-government agencies to promote self-management strategies for optimal health and wellbeing for all individuals. This position operates in collaboration with other members of the primary health team, including medical, nursing and Indigenous Health work force to deliver a holistic assessment, treatment, and self-management approach.

To be successful in this role you will have:

  • Works with the client, to provide culturally appropriate health education and health promotion activities aimed at increasing health literacy and health outcomes, whilst reducing the risk factors for chronic disease.
  • Manage a portfolio of Chronic Disease clients that would include but not limited to: Rheumatic Heart Disease, Chronic Kidney Disease.
  • To participate in primary health clinical services such as but not limited to:- health screening, disease prevention, health education, health promotion and community development in collaboration with other services and community as a means of improving the health and Closing the Gap between Aboriginal and Torres Strait Islander people and non-indigenous Australians.
  • Work with other health professionals to increase the number of completed health checks; inclusive of managing referral pathways for clients with chronic disease issues and ensuring that appropriate General Practitioner Management Plans and Team Care Arrangements are in place to maximise the health benefits to the individual.

Requirements for the role:

  • Appointment to this position requires proof of qualification and/or registration with the appropriate registration authority or association. Certified copies of the required information must be provided to the appropriate supervisor/manager, prior to commencement of clinical duties. Current registration with the Australian health Practice Registration Authority (APHRA) as a registered nurse and current practising certificate is mandatory.
  • This position requires the incumbent to operate a ‘C' Class Drivers Licence. An appropriate licence endorsement to operate this type of vehicle is required. Proof of this endorsement must be provided before commencement of duty.
  • It is a mandatory condition of employment in this role for the employee to be, and remain, vaccinated against the following vaccine preventable diseases during their employment: measles, mumps, rubella, varicella (chicken pox), pertussis (whooping cough) hepatitis B.
