About you:

In your new role you will…

• Demonstrate sound primary clinical skills in managing health care interventions through assessment, referral, treatment and monitoring stages, appropriate to the role.
• Demonstrate experience in the provision of specific chronic healthcare in client assessment, measurement, observations and treatment and deliver client focused chronic health education programs.
• Demonstrate good interpersonal skills and demonstrated ability to liaise and communicate with Aboriginal and Torres Strait Islander families and communities.

About your opportunity:

In this position you will provide culturally safe and effective clinical care and Liaison services to Aboriginal and Torres Strait Islander people while admitted to Atherton Hospital within a multidisciplinary team. The Health Worker is to work under minimal supervision, with full accountabilities and delegation of a Clinical Nurse to provide a high standard of care.

About your role:

• Screening high risk cases and follow-up clients in order to contribute to better health outcomes for Aboriginal and Torres Strait Islander people transitioning between hospital and community setting.
• Work in collaboration with other clinicians and services to enhance the health of Aboriginal and Torres Strait Islander people.
• Provide appropriate clinical support within the multidisciplinary team environment to ensure best health outcomes for Aboriginal and Torres Strait Islander people within the outreach communities.
