About you:

In your new role you will…

  • Demonstrate ability to work within a multidisciplinary team and utilise well developed communication skills to communicate with a wide variety of stakeholders (including clients and their families and carers, the multidisciplinary team, non-government agencies and other partner organisations).
  • Demonstrate ability to work autonomously and manage time effectively.
  • Demonstrate knowledge and (preferred) experience in case management.
  • Demonstrate ability to provide clinical supervision to subordinate personnel, commensurate with level of experience.

About your opportunity:

In this position you will deliver effective and efficient provision of occupational therapy services to older people, and their carers, partners and families, who have been discharged from acute and sub-acute care settings and accepted into the Transition Care Program. The role will include working collaboratively within a multidisciplinary team to provide occupational therapy and case management for allocated clients on the Transition Care Program.

About your role:

  • Provide consistent, safe and efficient occupational therapy and case management services, within a multidisciplinary team, with infrequent need for direct clinical practice supervision.
  • Provision of general clinical advice to professional and operational supervisors, relevant service managers and other stakeholders including non-government organisations and remote community agencies within Far North Queensland, regarding service delivery and case management on a regular basis and with infrequent need for supervision.
  • Providing ongoing development and maintenance of internal and external networks and proactively developing self, including maintaining contemporary knowledge of the profession, management and organisational strategies.
