About you:

You are a qualified Health Worker with ability to understand and respond to the cultural needs of the local Aboriginal and Torres Strait Islander community, you can communicate in individual and group situations within the workplace and the community. You are experienced in the provision of specific chronic healthcare in client assessment, measurement, observations, and treatment and deliver client focused chronic health education programs and you are competent to work independently and plan, prepare, deliver, and evaluate health promotion programs within appropriate set time frames.

Your opportunity: 

To provide a range of health services within a multidisciplinary health care setting both individually or under direct supervision, with full accountabilities and delegation of a Registered Nurse / CNC to provide a high standard of care, reviewing clinical run sheets, referrals, Emergency Department presentation lists, Post-acute, screen high risk cases and follow-up clients to contribute to better health outcomes for Aboriginal and Torres Strait Islander people transitioning between the hospital and community setting

Your role: 

  • Be self-directed and show initiative under the guidance of the Team Leader and Aboriginal and Torres Strait Islander Health Worker Cluster Coordinator as Professional Lead.
  • Establish and maintain a communication network between health providers, the Aboriginal and Torres Strait Islander community, and other services.
  • Identify health needs in adult health and chronic disease and participate in health promotion programs, appropriate referral, advice, and information.
  • Network and liaise with other service providers to ensure a coordinated approach to health service delivery.
  • Deliver culturally appropriate Aboriginal and Torres Strait Islander health information ensuring that services are provided in ways that maximise cultural acceptability and respect.
