About you:

In your new role you will…

  • Demonstrate ability to participate in community engagement activities, Aboriginal Islander Community Controlled Health Organisation (AICCHO) meetings and other community meetings to gather information to inform practice and service delivery and to impart knowledge to community groups.
  • Demonstrate ability to undertake administrative tasks including maintenance of records and data collection.
  • Demonstrate ability to work with indirect supervision and effectively manage and be responsible for own workload.

About your opportunity:

In this position you will work under minimal supervision with Rapid Response Falls Service within a multidisciplinary health care setting to provide clients with a high standard of support and assistance to self-manage aspects of their life that may include planning, access and involvement in community services and decision-making, delivery of specific health care programs, assessing and treating clients, maintaining health care systems and providing health education programs, assisting telehealth services and community liaising functions in order to contribute to better health outcomes for Aboriginal and Torres Strait Islander people.

About your role:

  • Act as a cultural liaison to advocate, liaise, network and refer to appropriate Non-Government Organisations, Aboriginal and Torres Strait Islander Liaison Officers and the Multidisciplinary Team in collaboration with and under direction of Registered Nurse.
  • Provide effective support and advocacy for identified Aboriginal and Torres Strait Islander Rapid Response Falls Service clients by conducting timely home visits, case management/conferencing, follow-up and referrals to relevant health services/programs.
  • Coordinate and conduct ongoing health promotion and education sessions with individuals and families within the local Aboriginal and Torres Strait Islander community. This may include but is not limited to, falls prevention, chronic disease, health screening, and general care for older people.
