Expression of Interest

The Nurse Navigation Service is a state-wide model of care designed to provide holistic support to patients with complex multiple health conditions e.g. chronic disease, mental health, frail and aged who require an expert level of healthcare coordination.

Nurse Navigators work across system boundaries to actively improve health literacy in close partnership with patients and families and other health and allied health services to improve the patient's healthcare experience and quality of life. A proactive and integrated approach is taken to ensure patients receive the most appropriate services where and when they need them.

The Nurse Navigator will work with patients who have complex conditions with a focus of maintaining wellness, avoiding hospital presentations and admissions, and reducing length of stay for those who are admitted to an acute facility.

This role requires a deep understanding of diabetes management, patient education, currency with diabetes medications and technologies and application of an individualised approach to addressing the unique needs and challenges faced by patients and the stakeholders in their care.

Key reponsibilities include:

  • Lead the achievement of positive patient outcomes by:
    • Applying expert clinical nursing knowledge and skills, and coordinating clinical practice across the healthcare continuum including facilitating pathways and access for patients to the most appropriate services in a timely manner.
    • Taking accountability and responsibility for ensuring that practice is evidence based and continually monitoring and evaluating nursing activity in the specialty area.
  • Contribute to the effectiveness of the multidisciplinary team through the provision of clinical nursing expertise and leadership.
  • Facilitate a learning environment by operationalising strategies that support and promote education, learning, and workforce development, including leadership in research initiatives.
  • Provide nursing leadership that drives system and quality improvement initiatives and change management.

Remuneration value up to $13,684.33 p.m., comprising salary between $5,081.10 - $5,516.60 p.f. (f/t), employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (Nurse Grade 7) (Fixed term temporary full time for periods of leave relief).
