About the role

  • The role delivers high level and quality, evidence based social work services as a member of the multidisciplinary team to those inpatient consumers who are transitioning from the adult acute inpatient units back to the community and who are not being followed up by the relevant continuing care team. This includes providing follow up care within seven days of discharge from the inpatient unit and ensuring consumers are linked into appropriate ongoing care. Services are focussed on optimising psychosocial outcomes by the early identification of psychosocial factors contributing to the admission and facilitating recovery from mental illness, supporting safe and timely discharge and reduced Length of Stay (LOS) through the provision of evidence-based interventions and appropriate referrals.

About you

Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:

  • Apply high level culturally competent Social Work clinical services for adult and psychogeriatric consumers with complex needs who are inpatients of Redcliffe Caboolture Mental Health Unit and their families/carers, in accordance with the Australian Association of Social Workers Professional Practice Standards and Code of Ethics.
  • Provide biopsychosocial assessments, mental state and risk assessments and timely social work interventions (individual and group) within identified models of care, including the Recovery Framework, to reduce the impact of psychosocial stressors/mental illness/alcohol and drug needs on consumer's functioning and lifestyle.
