About the role

Permanent full time opportunity to provide advanced level clinical social work services to patients and their families/caregivers who are accessing treatment through the RBWH Kidney Health Service.

This position will function as part of the multidisciplinary team, providing social work assessment and intervention, clinical leadership and consultation to staff at the RBWH and with outreach to community sites and satellite hospitals across Metro North Hospital and Health Service that come under the RBWH Kidney Health Service (including North Lakes, Kallangur, Bribie Island, Nundah and Stafford clinics).

About you

Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:

  • Exercise advanced level professional judgement, clinical reasoning skills, and knowledge of relevant legislation in the delivery of advanced level psychosocial assessment, evidence-based interventions and/or the facilitation of critical decisions for patients whose kidney disease may not respond favourably to treatments such as dialysis, have a high symptom burden or are considering withdrawal from dialysis.
  • Act as the primary point of contact for RBWH renal social work services and apply advanced level evidence and judgement in advising the Director Social Work, the Cancer Care and Chronic Disease Social Work Team Leader and other senior management, colleagues and relevant internal and external stakeholders regarding professional standards, complex clinical cases, legislation, management of resources and clinical service development.
  • Oversee, coordinate, and provide clinical leadership for RBWH Kidney Health social work services and less experienced social work practitioners to ensure best outcomes for patients and the wider community.
