Under the direction of the Manager, Clinical Operations or designate and in collaboration as a member of a multidisciplinary team; performs assessments, plans, implements and evaluates nursing care. The LPN operates in accordance with the competency guidelines and full scope of practice within the Standards of Practice as outlined by the British Columbia College of Nurses and Midwives (BCCNM), and according to agency operating policies and standards and unit specific protocols. Provides client and family-centered care through a trauma-informed approach and evidence informed practice. Encourages knowledge exchange in day-to day activities, empowers clients and natural supports to engage and connect within their communities, and ensures they are supported in navigating the health care system. May provide care to patients through a combination of telephone, virtual video/audio conference and/or remote technology to patients in their homes, in the community including outreach, and virtually.
Education, Training & Experience
Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Graduate of a recognized practical nursing program or equivalent combination of education, training and experience.
One (1) year related clinical experience. Completion of or eligibility to complete Mental Health & Substance Use Competency Education.
Current Class 5 Driver’s License and access to personal vehicle for business related purposes, as required.
