Due to continued growth at the Surgery we are seeking a dedicated individual to join the team as a Care Navigator. As a Care Navigator, you'll play a pivotal role in assisting patients, providing administrative support, and facilitating effective communication between healthcare professionals and patients.

Key Responsibilities:

  • Patient Assistance:Receive, assist, and direct patients to appropriate appointments and services with a courteous and efficient approach.
  • Administrative Support:Undertake various administrative duties to ensure the smooth running of the practice, including clerical support and record-keeping.
  • Communication Facilitation:Serve as a liaison between patients, primary healthcare teams, secondary care, and other healthcare agencies to ensure effective communication.
  • Appointment Management:Process appointment requests, handle telephone consultations, and ensure urgent cases receive timely attention.
  • Confidentiality Maintenance:Uphold strict confidentiality standards regarding patient information and practice-related matters.
  • Health & Safety:Promote and maintain health, safety, and security within the workplace according to practice policies and procedures.
  • Equality and Diversity:Support equality, diversity, and patient rights by respecting privacy, dignity, and individual needs.
  • Personal/Professional Development:Actively participate in training programs and maintain personal and professional development records.
  • Quality Assurance:Contribute to maintaining quality within the practice by assessing performance, identifying risks, and suggesting improvements.
  • Effective Communication:Recognize the importance of clear communication within the team, with patients, and with other stakeholders.

Qualifications:

  • Previous experience in a healthcare or administrative role is preferred.
  • Excellent communication and interpersonal skills.
  • Strong organizational abilities and attention to detail.
  • Ability to work independently and as part of a team.
  • Commitment to maintaining confidentiality and upholding ethical standards.
