Clinical Care Coordinator

DFP Recruitment is seeking a Clinical Care Coordinator to join our Federal Government clients team. This roleis full time hours (Mon - Fri) and will be based inTownsville.
The position ofClinical Care Coordinator entails working collaboratively with clients and internal and external stakeholders to enhance the ability to identify and respond to the needs of vulnerable and at-risk veterans, current & former Defence Force members and/or their families.
The Clinical Care Coordinator will take a strengths-based, recover approach to supporting vulnerable and/or at-risk clients to enhance wellbeing and participation in their community through community engagement, collaborative recovery work, case coordination and complex case management.
The role requires you to:

  • Liaise and form partnerships with various internal and external stakeholders
  • Direct provision of high quality, evidence informed clinical services including intake, assessment and referral, complex case management, and case coordination.
  • Maintain clinical records, collating and analysing data from client records & preparing reports as required
  • Work systemically with key internal and external stakeholders to influence a recovery approach within a stepped care model for vulnerable or at-risk veteran and defence force communities and families
  • Participate in community development and service promotion activities
  • Contribute to the development and delivery of group programs
About you:
  • Degree qualified Registered Mental Health Nurse, Psychologist, Social Worker or Mental Health Occupational Therapist
  • Full registration with AHPRA or AASW (eligible for MH Accreditation)
  • Working with Children (Blue Card) or Working with Vulnerable People (Yellow Card)
  • AnAustralian Citizen and willing to undertake a National Police Clearance.

  • An attractive hourly rate of$65 per hour + super
  • 12 months
  • Monday to Friday, 37.5 hours per week
  • Hybrid model (2 days from home)
