About you:

You are a qualified Pharmacist with general level of knowledge, expertise, and skill sufficient to provide safe routine pharmaceutical care to patients in a hospital setting across a variety of clinical teams and ability to provide routine professional advice in the context of multidisciplinary team participation. You are willing to participate as a member of the Pharmacy Team with flexibility to respond appropriately to a variety of needs and can provide guidance to less experienced staff members.

Your opportunity: 

This position is a First Nations Graduate Pathway position providing a culturally safe and supportive entry to practice for allied health graduates of identified First Nations background. The position will provide the patients of the Cairns and Hinterland Hospital and Health Service with best practice pharmaceutical care in accordance with their needs and the policies and priorities of the Health Service. The common objective of the Pharmacy Service is that patients derive maximum benefit from medications whilst being exposed to minimum risk. This position will receive enhanced training, development, and cultural mentoring support with Indigenous Allied Health Australia (IAHA) Leadership Program during first 12 months.

Your role: 

  • This position will function as part of a team of pharmacists providing services to patients within the Pharmacy Services of the Cairns and Hinterland Hospital and Health Service
  • An expected outcome of this position is to provide accurate and timely advice to medical, nursing, and subordinate pharmacy staff on patient admission history and pharmaceutical issues while ensuring optimum patient medication safety.
  • Deliver general level clinical pharmacy services within the Community Services team as part of a multidisciplinary team in accordance with prescribed professional and ethical standards and under the direction of the Senior Pharmacist Community Services.
  • Undertake routine professional pharmacy tasks (including taking a medication history upon admission, inpatient chart review, medication action planning, medication discharge planning, provision of discharge medication record, dispensing and counselling, liaison with community providers, medication safety improvement initiatives, periodic audits of prescribing, supply and controlled drug records, and other agreed key performance indicators) within established protocols.
